When you register for an Initail e-mail consultation, RDC will e-mail you a detailed assessment form to fill out. This form will include tips and hints on how to answer the questions, which will help RDC provide you with recommendations suitable to your unique lifestyle. Information collected on this form includes details of:

• Anthropometry (measurements such as weight, height and waist circumference)
• Medical history
• Lifestyle and social information
• Diet history

Once you return this form to RDC, Rebecca will assess the information provided and offer you practical recommendations and a realistic plan of action to improve your nutritional status, including supporting education resources.

Following your Initial E-Consultation you have the option of registering for an e-mail review consultation, in which RDC will reassess the dietary and lifestyle changes you have made.
